As a general surgeon working in Nuwara Eliya, Sri Lanka, I've encountered numerous patients suffering from varicose veins and venous ulcers. These conditions are especially prevalent among manual workers and business workers who spend long hours standing or lifting weights.
Understanding Varicose Veins
When we stand for prolonged periods or strain our bodies, the pressure within the veins of the lower limbs increases. This increased pressure can cause the valves in the veins to malfunction, leading to blood pooling and vein dilation. Over time, this can result in microperforations, skin discoloration, and venous ulcers.
Importance of Early Diagnosis and Treatment
Early diagnosis and treatment of venous disease are crucial to prevent further complications. Before proceeding with any treatment, a venous duplex scan is recommended to check for blood clots, as these need to be addressed first to avoid potentially life-threatening complications.
Treatment Options
For varicose veins, particularly when the malfunctioning valve is above the knee, endovenous laser ablation (EVLA) is a preferred method. This minimally invasive procedure involves:
- Pre-Procedure Preparation: The patient is given a local anesthetic, and the area is sterilized.
- Procedure Steps: A catheter is inserted into the affected vein, and a laser fiber is used to deliver controlled heat. This heat causes the vein walls to collapse and seal shut.
- Post-Procedure Care: After the procedure, patients are advised to wear compression stockings for two weeks. This helps reduce swelling and supports healing.
Potential Complications
While EVLA is generally safe, patients might experience:
- Skin Discoloration: Temporary changes in skin color around the treated area.
- Thrombophlebitis: Mild tenderness or pain, which can be managed with simple analgesics.
By highlighting the importance of early diagnosis and advanced treatments like EVLA, we can better manage and treat varicose veins, improving the quality of life for those affected.
